I can't take HRT (I'm a cancer survivor). How do I handle perimenopause fat gain without it?
With the same foundation we coach every woman on: strength training, enough food and protein, sleep and recovery, and a coach who adjusts the plan as you go. None of that depends on HRT. Your oncology team leads on anything medical, and we build your training and food around what they've cleared.
There's a lot you can do here.
It's the part we coach every day.
And you won't be figuring it out alone.
First, when should you talk to your doctor?
Before you start, and any time something changes. If you've been through cancer treatment, that means your oncology team too.
We coach. We don't diagnose, treat or give medical advice.
- Before you start any new training program. Get their clearance. If they've given you limits, your coach works within them.
- About anything hormonal or medical. HRT, medications and your cancer care are a conversation for your oncologist and your doctor.
- Before you add any supplement or herb. With us, food comes first and supplements are always optional. Your care team checks anything new against your health and your medications.
- If they've given you food guidance. If your care team has asked you to eat or avoid certain foods, send it to your coach. Your plan gets built inside it.
Here's our side of it.
We coach food, training, sleep and habits around what your doctor says.
We'll never tell you to start, stop or change any medication or HRT.
Your health care is their job.
Ours is helping you get strong and feel like yourself in your body again.
Who is this page for?
Maybe you're a cancer survivor and HRT isn't an option for you.
Maybe your doctor said no for another reason.
Maybe you tried it and it wasn't right for you.
Or maybe you just don't want it.
All of those are okay. Some of the women we coach are on HRT. Some are thinking about it. Some can't take it, or don't want it. We coach all three, and you can see how on our perimenopause fat loss coaching page.
And if you are on HRT and still stuck, here's what women on HRT are usually missing.
What's changing in your body?
A lot of the women we talk to describe the same things:
- Fat settling around the middle. Even when nothing about how they eat has changed.
- Feeling weaker. Things that used to be easy take more effort.
- Sleep that isn't what it was. Waking up tired, or waking up at night.
- Energy that runs out early. Fine in the morning, done by mid-afternoon.
Some of the women we talk to tell us treatment brought on menopause suddenly, and the changes showed up almost overnight.
If you want the longer explanation, read why you're gaining belly fat in perimenopause when nothing changed.
Here's the thing.
In our experience, the pieces that change body composition are the ones you work on every day: how you train, how you eat and how you recover. That's the same with HRT or without it.
Most women come to us wanting to lose weight. What they actually want to lose is fat.
So we focus on losing fat and building strength, and we watch a lot more than the scale. How your clothes fit. How strong you feel. How you're sleeping.
What does the plan look like without HRT?
The same foundation every woman gets, built around you:
- Strength training. Follow-along workouts at home, about 50 minutes, that build week to week. You need a set of dumbbells, a Swiss ball and fabric bands. Chris calls resistance training "the foundation and heartbeat" of everything we do.
- Enough food and enough protein. Real food, built around what you enjoy and around anything your care team has asked of you. No calorie counting, no food scales, no macro tracking.
- Sleep and recovery. Built into the plan from the start, because training only works when your body gets the chance to recover from it.
- A coach who adjusts. A PN-certified coach who is a woman over 40 and has done the program herself. A check-in every two weeks, plus direct access to her between them.
It's the everyday stuff, done consistently, with someone watching how your body responds.
What if my energy isn't what it used to be?
That's real, and we plan for it.
A lot of the women we talk to say their energy after treatment comes and goes. Some weeks you'll feel strong. Some weeks you won't.
- Every exercise has an easier option. Chris coaches through the videos with modifications and alternatives, so you can scale any move to the day you're having.
- The plan flexes. A low-energy week can simply be a lighter week. Your coach adjusts it with you.
- You start where you are. Your coach meets you at today's starting point and builds from there.
- You tell us how it's going. Chris says it to every woman who joins: "All we want from you is communication." If something doesn't feel right, tell your coach, and check with your care team.
Gentle progress still counts.
Showing up on the harder weeks counts most of all.
Can women who've been through cancer really do this?
Yes.
We coach women who've been through cancer treatment, and women who can't take HRT for other reasons.
Chris explains it like this: it can be done, and the women who've gone through it with us are grateful they did.
Some of the women we talk to who've been through treatment tell us they just want their body back.
Strong. Capable. Theirs again.
That's a very reasonable thing to want. And it's exactly what we coach.
We've helped 1,000+ women lose 10 to 20 lbs of fat in 8 weeks.
Ready to talk it through?
Your care team handles your health.
We'll handle the training, the food and the check-ins around it.
Start with the short video on our homepage at fortyfwd.com. It walks through why your body stopped responding after 40 and the method we use to change that.
If it sounds like you, apply for a call with Chris.
He reads every application himself. On the call, he'll listen to your history, ask what your care team has cleared, and tell you straight whether the program is the right fit. If it is, he walks you through the investment then, once you both know it's a fit.
You want your body back.
Let's start building it.
Frequently asked questions
Can I lose weight in perimenopause without HRT?
Yes. In our experience, strength training, enough food and protein, sleep and recovery, with a coach who adjusts the plan, are what change body composition, on HRT or off it.
I'm a cancer survivor. Can I start strength training?
That's a question for your oncology team, so get their clearance before you start anything new. Once they've cleared you, your coach builds the plan within any limits they've set, and every exercise has an easier option.
Do you recommend supplements, herbs or foods to balance hormones?
No. Nothing we recommend is meant to balance hormones, and supplements are always optional. Your nutrition plan is built on real food and any guidance your care team has given you, and any supplement or herb question goes to your oncologist and your doctor first.
What if my energy is low after treatment?
Tell your coach. The plan flexes week to week, so a low-energy week can simply be a lighter week, and if something doesn't feel right, check with your care team.
Do you work with women who don't want to take HRT?
Yes. Some of the women we coach are on HRT, some can't take it and some don't want it. The foundation is the same for all of them.
